Joining the Dots...Creating Connections between Communities and Primary Care

Join us for this year's AGM and networking event...
Building strong, respectful and long lasting connections between local communities and Primary Care has never been more important. We are all living longer, but many of us will develop complex issues and conditions as we get older which will inevitably lead to greater pressure on primary and secondary care (i.e. GPs, Pharmacists, community nursing and hospitals).

Our health is also determined by other factors such as income levels, our education, even the amount of social connection and support we receive from family and friends. Our traditional model dictates that when you are ill, you visit the GP but with only a ten minute slot per person, can we expect GPs to understand and solve these complex issues which often have their roots in social issues, not medical?

There is another way and at our workshop you will hear from two different community based organisations - Resurgam Trust and Mid & East Antrim Agewell Partnership - who are leading the way in joining the dots with their local primary care colleagues. They will share with us their insights into what works and just as importantly, what doesn’t work, when building and maintaining connections. We will have time to discuss and debate the many challenges and opportunities the current health crisis in NI presents us with and how developing 'health literacy' skills might be one of the keys to change.

We are also delighted to be launching the Self Care Pharmacy project at the event.
